A parent or legal guardian is required for the first visit (please bring guardianship papers if you are not the parent), but a letter of permission can be written to let someone else accompany the patient to any follow-up appointments.

Where may I park?
You may park in sub-level 1 or 2 at the Methodist Plaza parking garage, which can be accessed from Floyd Curl or Ewing Halsell. There is a fee for parking longer than 30 minutes. We are not able to validate parking.
If a cast is required, what color options are available?
Choose from black, maroon, purple, blue, light blue, red, pink, orange, green, light green, yellow, white and camouflage. Waterproof liners for your cast are optional, and insurance companies do not pay for the extra charge.

What can I do if my insurance is not accepted?
We have many contracts with insurance companies. If you do not find your insurance plan on the list, you can call our office for options.
Am I responsible for charges not covered by the insurance company?
Yes, all charges not covered by insurance become your responsibility.
When are co-pays, coinsurance and deductibles due?
Payments are collected at time of service.

When should I present proof of insurance?
You must present all proof of insurance coverage at time of appointment. Failure to do so may result in claims not filed to the insurance company, and you will be responsible for total charges.
Whom do I call for questions regarding my bill or insurance claim?

Do you accept auto or third-party insurance?
We do not accept auto or third-party insurance as a form of payment. We will provide the information needed in order to file a claim.
How do I request an itemized statement?
If your insurance company requests an itemized statement, please fill out the Records Release form. Once completed, either fax it to 210-616-0290 or bring it into the office.
