Records Request

To request your medical records, please click the “Request Your Records” button below and complete the authorization form. Once reviewed, you will receive an email from Pediatrix with a link to enroll in the patient portal, which will allow you access to the requested medical records. Upon receipt of this email, you will have 30 days to retrieve your records, at which point your access will be disabled. If you are unable to retrieve your records within the 30-day period, a new request can be submitted electronically following the same process.
