Appointment scheduling

You may complete an appointment request form electronically and someone from our office will call you to schedule and confirm your appointment. Our office is open Monday through Thursday from 7:30 a.m. to 4:30 p.m. and Friday from 7:30 a.m. to noon. A referral by your pediatrician is strongly recommended. An appointment for a routine evaluation can usually be obtained quickly, often within the week. For urgent appointments, please call the office at 407-253-1000 and press No. 1 to speak with someone directly.

Patient registration

Prior to your appointment, please complete the patient registration and medical history forms, available in the Phreesia app.

Preparation/what to bring

  • Please bring any authorizations and prescriptions. Your appointment may be rescheduled if you do not have proper authorization to be seen.
  • Please bring your current insurance card and photo ID.
  • Review other supplemental informational forms provided for you.

Cancellations

Please call us at least 24 hours in advance if you need to reschedule or cancel your appointment. This will allow another child to be seen in a timely fashion.

Hearing Problems

If you think your child has a hearing-related problem or may need a hearing test, please notify the receptionist at the time of your call or online appointment request. A separate appointment with the audiologist will be arranged. Some insurance companies consider the audiologist as a separate care provider and may require us to charge a second copay.

X-Rays

If you think X-rays are needed for the specialist to properly evaluate your child, please let the receptionist know at the time of your call. We may be able to obtain them prior to your appointment.
